Insurance
All payments are due at the time of service and the sole responsibility of the client.
I am currently in-network with Cigna, Optum, UnitedHealthcare, UnitedHealthcare Shared Services (UHSS), GEHA - UnitedHealthcare Shared Services (UHSS), UnitedHealthcare Global, UnitedHealthCare Exchange Plans (ONEX), Oscar, Harvard Pilgrim, Oxford, UHC Student Resources, UMR, All Savers (UHC), Health Plans Inc, Surest (Formerly Bind), Aetna, Meritain, Nippon, Allied Benefit Systems, GEHA - (UHSS), Trustmark, Trustmark Small Business Benefits, Health Scope, and Christian Brothers Services only.
Many insurance companies do provide an out of network policy, allowing you to choose the provider you want, with the option to be reimbursed if you provide them with a receipt of service. I can provide you with a receipt after you pay in full at the time of service. Superbills are sent out on a monthly basis on or after the 15th for the previous month and include all information needed for you to request reimbursement from your insurer for covered services. LTS does not and will not submit bills to insurance companies on clients' behalf for reimbursement. All insurance reimbursements for services will be the clients responsibility.
Please confirm your coverage, benefits and reimbursement process prior to your first appointment and arrive prepared to pay for your session.
Payments
Payment is expected at the time of service. For teletherapy, the session will be charged ahead of the first appointment.

IvyPay is a secure HIPAA-compliant credit card processing service. their website is: https://www.talktoivy.com/ivypay
To use I will enter your cell phone number along with a charge for the session fee. Ivy Pay texts you a secure link leading to a page where you enter your credit card information that is stored on their servers only, and approve the first charge. After future sessions, I use Ivy Pay to charge the stored card; the app sends you a text informing you that the payment has been completed. You will only be asked to enter your credit card information once, unless you need to update the card on file at any time.
Cancellation Policy
Your appointment time is reserved specifically for you. If you are unable to keep your scheduled appointment, or need to reschedule for any reason, I require 24 hours notice (72 hour for Monday sessions). Cancellations must be made either by phone or email. With this advanced notice, I have the opportunity to fill your open session. Without notice Cancellations made within 24 hours (72 hour for Monday sessions) of the appointment time are charged a rate of $45. This cancellation fee may be waived if we are able to reschedule the appointment within the same M-F week, depending on mutual availability.
No-shows to appointments, cancellations occurring within 24- hours of the start time or after the appointment start time are charged at a rate of $45, regardless of availability to reschedule within the M-F week.
If I need to cancel our appointment within the 24 hour window, I offer a no-charge late-cancel credit.
Client Forms
For your convenience, all required forms will be accessible through your client portal upon scheduling. Individually signed forms are needed for each counseling participant. If you are a part of a couple or family seeking therapy together, you will each need to complete your own forms.
For parents: I will need a custody agreement before beginning therapy if one is on file with the courts.

Good Faith Estimate
Under Section 2799B-6 of the Public Health Service Act, as of 1/1/2022 health care providers and health care facilities are required to inform individuals who are not enrolled in a plan or coverage or a Federal health care program, or not seeking to file a claim with their plan or coverage both orally and in writing of their ability, upon request or at the time of scheduling health care items and services, to receive a “Good Faith Estimate” of expected charges.
A provider’s duty to provide notice and a GFE applies to self-pay clients, i.e., an individual who (1) does not have benefits for an item or service under a group health plan, group or individual health insurance coverage offered by a health insurance issuer, federal healthcare program, or a health benefits plan; or (2) chooses not to use his or her coverage benefit for the item or service.
Individuals enrolled in short-term, limited-duration health plans or other types of products not regulated as health insurance coverage (e.g., health-sharing ministries) are considered self-pay clients.
If an individual’s plan or coverage provides no benefit for out-of-network services, the individual would be a self-pay client for any out-of-network provider. If, however, the plan or coverage provides a limited benefit (e.g., higher co-insurance items or services furnished by out-of-network providers), the individual would not be a self-pay client (unless the individual chose not to use such coverage benefit for the item or service).
You will receive this estimate prior to attending your first scheduled appointment if you are eligible as a self-pay client. All services and fees are clearly stated in our Informed Consent new client paperwork for you to review prior to your appointment. I will also discuss these fees with you prior to and during your initial appointment, and answer any questions you may have related to any applicable fees.
Location
All in-person sessions are provided at 8603 Crownhill Boulevard, Suite 25, San Antonio, TX 78209, in the El Cid Building on the second floor, accessible by stairs only. A map is available here. All teletherapy sessions are provided online through a secure HIPAA-Compliant platform and can be provided to any resident of Texas. Some insurances are now excluding telehealth from reimbursement, so please check with your insurance provider to be sure that they will cover services if you plan on attending via telehealth.
