- New Client Interest Form -

Thank you for your interest in my practice. Please fill out and submit this form, and I will respond via email within two business days.

Please mark possible availability for sessions below. Openings are limited, and you may view them on the calendar under Client Portal. If you do not see a slot that works for you but are interested in working with me, feel free to still mark your availability below & we will see what we can work out.
What is your desired frequency for sessions? I do not currently take new clients who wish to be seen less frequently than every-other week. After we work together for some time, we may decrease your frequency to an "as-needed" or "monthly maintenance" frequency.
  1. Our therapeutic journey will involve delving into your past relationships and experiences to gain insight into your current behavioral patterns. If you're not comfortable exploring your past, I may not be the right therapist for you.

  2. Our focus will be on facilitating emotional processing by aiding you in recognizing, comprehending, and experiencing your emotions. If delving into emotions isn't something you're interested in, I might not be the right therapist for you.

  3. Collaboration is at the heart of our work together. I don't provide direct answers or advice. I respect your expertise in your own life, and you will guide our sessions with your insights and preferences. If you seek a therapist who directs the process and offers solutions, I may not be the right fit for you.

  4. Our journey towards exploration and healing is a long-term endeavor. While we'll address immediate stressors, please understand that healing takes time. If you're seeking a therapist who provides instant solutions or a checklist of tools, I may not be the right therapist for you, and that is okay. There are many different approaches and I encourage you to find a therapist that best fits with your needs.

The following information explains payment for services: 

  • My rate is $130 per 50-minute session. 

  • I accept some insurances via Alma. A list of those insurances can be found on the Policies page.

  • You will be responsible for a $45 fee if you cancel less than 24 hours prior to your scheduled session (72 hours for Monday appointments).

  • Before our first session I will send you forms to enter your credit card information/insurance information. A current and valid form of payment must remain on file during our work together. I'll securely store those payment details and on the morning of each session, a charge will be made for the full payment of your session.

  • If you choose to pay via insurance, it is your responsibility to understand your copayments, co-insurance, and benefits. 

  • If you are experiencing financial setbacks, please communicate that with me so that we can find a solution before sessions.
